Who can refer?

Referrals can be made by system partners or Service Users can choose to self-refer. Partners include;

• Primary care

• Substance Misuse/Addiction services

• Housing Support services

• Carers Support services

• Mental Health services such as NSFT

• VCFSE service

• Norfolk and Suffolk Constabulary

• Wellbeing Hubs

• Social Care

• Local Authorities and District Councils

 

What can Service Users expect from our team?

We will:

• Carry out an initial needs assessment

• Agree an individual support plan which addresses their needs

• Hold meetings for regular reviews and assessment of progress

• Support to access other services that will help, such as counselling, drop-in spaces, group work and signposting you other therapies which may help

• Ask for feedback regularly and suggestions as to how we can improve the service

• Improved physical health

• An improvement in quality of life, mental health and wellbeing

• An increase in engagement with the mental health and/or substance misuse/addiction services needed

• A reduction in the negative effect of having a dual diagnosis
