We deliver the Community Pathway Service across Suffolk, working closely with Woodlands Hospital in Ipswich and the community mental health teams.

 

Our community-based offer provides a pathway discharge service for those leaving hospital, or preventative support before admission, to assist with re-engagement into the community to reduce the need for first or further hospital interventions.

 

We will offer 6 weeks of community and home support, to help individuals reduce their risks of crisis, through settling back home after a hospital stay, finding activities, group community sessions, encouragement to work towards specific goals and to help improve their mental health and wellbeing.  

 

The combination of the current Community Connector and the new Pathways Support service will demonstrate a holistic and joined up approach to support an individual’s recovery.
